Details with respect to plan benefits, claim procedures and limitations can be found in the Summary Plan Description. Here's a summary of what's covered. Please keep in mind that the plan pays after deductibles are met:

Emergency room services $100 co-pay-waived if admitted
Urgent Care Please see your regular doctor for routine care
Ambulance services 80% co-insurance in- or out-of-network
Durable medical equipment/prosthetics $5,000/$25,000 maximum
Hospice facility/home hospice $10,000 individual maximum, includes
bereavement counseling
Skilled nursing facility 30 days per year
Physician office visits $25 co-pay per visit
Routine and preventive services $150 maximum. Includes vaccinations, inoculations
and immunization for dependents age 1-19
Vision exams $25 co-pay. 1 per year
Well child check-ups till age 1 $25 co-pay
Inpatient hospital services $200 co-pay
Outpatient surgery $200 co-pay
Outpatient therapy services 60 combined visits per year
Chiropractic services 10 visits per year
Home healthcare 40 visits per year
Inpatient mental, nervous and substance abuse treatment Combined 30 days per year
Outpatient mental, nervous and substance abuse treatment $25 co-pay. 15 visits per year
Transplants United Healthcare Transplant Network

If you go to doctors and facilities within the United Healthcare Choice Plus Network, the plan covers these benefits up to 80% after deductible and co-payments are applied. After you reach the coinsurance maximum, the benefits are covered up to 100%.

If you go to doctors and facilities outside of the network, the plan will only cover up to 60% of the cost after deductibles and copays are met. But, your 40% co-insurance may not be your only cost (see page 10 for an example).

Our rates reflect a discount for non-tobacco users. Smokers pay more per week. The medical evidence is irrefutable-tobacco use increases medical costs! We reward our employees and families who choose not to use tobacco.

Rates are reduced on your first anniversary! The reduction is $3/week for single coverage or $6/week for family or single + coverage.
