If you go to doctors and facilities within the United Healthcare Choice Plus Network, the plan covers these benefits up to 80% after deductible and co-payments are applied. After you reach the coinsurance maximum, the benefits are covered up to 100%. If you go to doctors and facilities outside of the network, the plan will only cover up to 60% of the cost after deductibles and copays are met. But, your 40% co-insurance may not be your only cost (see page 10 for an example). Our rates reflect a discount for non-tobacco users. Smokers pay more per week. The medical evidence is irrefutable-tobacco use increases medical costs! We reward our employees and families who choose not to use tobacco. Rates are reduced on your first anniversary! The reduction is $3/week for single coverage or $6/week for family or single + coverage. |
